COVID-19 Reporting form

At finao, we understand that we must anticipate and respond fast to requirements. This is especially at the forefront of our efforts now, in the face of the rapidly unfolding events of the COVID-19 pandemic. The health, safety and wellbeing of the community is important to us and to our clients. We have developed a COVID-19 Reporting form which provides a standardised approach to recording the details of potential COVID-19 cases and deciding on next steps.

The COVID-19 Reporting form allows you to keep a record of key information of potentially affected individuals. Some of these key details include, but are not limited to;

  • Presenting symptoms
  • Travel history in the past 14 days
  • Whether the affected person has been to a hospital, seen a doctor, and been tested
  • Whether they had close contact with other employees/contractors, or people with certain existing conditions
  • Next steps

This data is essential for monitoring, decision-making and managing an appropriate response to the risk of COVID-19, especially as the picture changes on a daily basis.
