We have developed a COVID-19 Reporting form which provides a standardised approach to recording the details of potential COVID-19 cases and deciding on next steps.

The COVID-19 Reporting form  allows you to keep a record of key information of potentially affected individuals. Some of these key details include, but are not limited to;

• Presenting symptoms
• Travel history in the past 14 days
• Whether the affected person has been to a hospital, seen a doctor, and been tested
• Whether they had close contact with other employees/contractors, or people with certain existing conditions
• Next steps

This data is essential for monitoring, decision-making and managing an appropriate response to the risk of COVID-19, especially as the picture changes on a daily basis.
